Prior to your first appointment, through email, you will receive patient intake forms to complete. These will include demographic, pricing, policy, health history, privacy and practitioner specific consent documents.
Upon visiting one of our sites, you can expect to be greeted by a friendly and helpful administrative or treatment staff member who will perform a temperature check and ensure that you have completed and passed all of our COVID-19 safety protocols and screening questionnaire. You will then undergo a detailed health history and physical examination by a practitioner in order to properly diagnose your problem and subsequently determine the best treatment plan for your health goals that may include the addition of other healthcare practitioners.
Once your diagnosis and treatment plan have been outlined to you and all questions addressed, treatment will commence unless you are recommended for additional special testing for additional evaluation.
